I understand that the registration fee and/or materials fee are non-refundable.*

I understand that tuition is due the first day of class. An invoice will be sent via email with a link to pay. After a 5 day grace period, there will be a $10 fee added to late payments.

If paying via a third party, it is your responsibility to ensure that payment is still submitted to the studio in a timely manner. Late fees will still apply if third party payments are not received by the due date.*

All classes involve physical movement and activity. Accidents can happen and there is a risk of injury. By registering in the program, the parent or legal guardian accepts full financial responsibility for any accident or injury that may happen to their child, themselves, or any guest accompanying them while at Trimble Music Studio, and anywhere on the premises.

 You agree to release, hold harmless, and indemnify Let's Play Music, Trimble Music Studio, and Jennifer Trimble from any claims.*

I grant permission to Let’s Play Music, Trimble Music Studio, and Jennifer Trimble to use mine and/or my child’s image (photographs and/or video, including electronic versions) throughout the program for use in events including but not limited to videos, newsletters, emails, and social media for the purpose of training, certification, and marketing. I hereby waive my right to inspect or approve the finished media. I also waive my right to any royalties or other compensation.
