I understand that both this registration form and the enrollment with the Let's Play Music Site are necessary to hold my child's spot in class.

I understand that the registration fee and the materials fee are both non-refundable.*

I understand the importance of participating in the parent education that will be sent via email over the summer. I will complete the parent orientation prior to the first day of class.*

I understand that a parent is required to attend class with their child twice a month for the first year.*

I understand that siblings, including babies, are not allowed in class.*

I understand that Let's Play Music is a 3 year program. It is anticipated that my child will complete the entire 3 year program with Jennifer Trimble as their teacher.

When a student drops from the program, their spot cannot be filled, leaving a vacancy for the remaining 3 years. To protect the studio against this loss, there is an early withdrawl fee of $40 per uncompleted semester.

Please communicate with your teacher about job loss, extreme illness, or moving.

I understand and agree to the following tuition schedule:

• (2025) Year 1 - 45 minute classes: $560

• (2026) Year 2 - 50 minute classes: $640

• (2027) Year 3 - 55 minute classes: $720

If needed, you may pay the yearly tuition by semester (half) or monthly (divided by 8 payments). If dividing tuition, installments must be made by automatic payment through the provided link. There are 15 lessons in each semester. Tuition is not discounted, prorated or refunded due to missed lessons.*

Tuition is due the first day of class. After a 5 day grace period, there will be a $10 fee added to late payments. This also applies to monthly and/or semester payments that are not received on time.

If paying via a third party, it is your responsibility to ensure that payment is still submitted to the studio in a timely manner. Late fees will still apply if third party payments are not received by the due date.*

Let's Play Music involves physical movement and activity. Accidents can happen and there is a risk of injury. By registering in the program, the parent or legal guardian accepts full financial responsibility for any accident or injury that may happen to their child, themselves, or any guest accompanying them while at Trimble Music Studio, and anywhere on the premises.

 You agree to release, hold harmless, and indemnify Let's Play Music, Trimble Music Studio, and Jennifer Trimble from any claims.*

I grant permission to Let’s Play Music, Trimble Music Studio, and Jennifer Trimble to use mine and/or my child’s image (photographs and/or video, including electronic versions) throughout the program for use in events including but not limited to videos, newsletters, emails, and social media for the purpose of training, certification, and marketing. I hereby waive my right to inspect or approve the finished media. I also waive my right to any royalties or other compensation.
